A hypertensive crisis — a sudden, severe spike in blood pressure, generally systolic above 180 and/or diastolic above 120 — is a distinct situation from the gradual, usually symptomless high blood pressure most people live with.
Hypertensive Urgency vs. Emergency
- Hypertensive urgency: severely elevated blood pressure without signs of organ damage. This still needs prompt medical attention, typically within hours, though not always via emergency room.
- Hypertensive emergency: severely elevated blood pressure WITH signs of organ damage — this requires immediate emergency treatment.
Warning Signs of a Hypertensive Emergency
- Severe headache
- Chest pain
- Shortness of breath
- Vision changes
- Confusion or difficulty speaking
- Numbness or weakness, especially on one side of the body
- Severe anxiety or a sense that something is seriously wrong
What to Do
If you or someone else has a very high blood pressure reading along with any of these symptoms, seek emergency medical care immediately — call emergency services rather than driving yourself. If a high reading occurs without these symptoms, retake the reading after a few minutes to confirm it, then contact a doctor promptly rather than panicking — but this still needs medical follow-up rather than being ignored.
