Treatment for H. pylori

H. pylori is treated with a combination of medications — typically two antibiotics along with a medication that reduces stomach acid, and sometimes a fourth medication containing bismuth — taken together for one to two weeks. Doctors and pharmacists often call these combinations "triple therapy" (three medications) or "quadruple therapy" (four, when bismuth is included). Combination therapy is used because it's more effective than a single antibiotic at fully clearing the infection and reduces the risk of antibiotic resistance; because resistance has been rising, many current treatment guidelines now favor a full 14-day course rather than a shorter one.

Why Combination Therapy Is Used

A single medication is less likely to eliminate H. pylori completely, and incomplete treatment can allow the bacteria to develop resistance, making future treatment more difficult. This is why treatment courses involve multiple medications taken exactly as prescribed for the full course.

Confirming It's Gone

After completing treatment, a follow-up breath or stool test (not a blood test, which can stay positive even after successful treatment) is typically done at least four weeks later to confirm the infection has actually cleared. Testing too soon, or while still taking acid-reducing medication or antibiotics, can produce a false-negative result — your doctor will tell you how long to stop these before the follow-up test, commonly around two weeks off acid-reducing medication and four weeks off antibiotics.

Not Everyone With H. pylori Needs Treatment

Treatment decisions depend on symptoms, ulcer history, and other individual factors — not everyone who tests positive for H. pylori is automatically treated, so this is a decision made with a doctor based on your specific situation.
